House of Transformations Transitional housing serves individuals with addiction, criminal convictions, homelessness in a group living environment. It is a client-centered housing and supportive services program that provides individuals with case management, peer support, life skills, employment specialist, support services with information and referrals to the community resources. The primary goal of the program is to prepare individuals to long term sufficiency: stable and independent of ongoing support.
